Introducing Bradford Children in Care Nursing Team

The Children in Care Team support all children and young people in Bradford who are in care, up the age of 18years. It is run by Bradford District Care NHS Foundation Trust (BDCFT).

What do the team do?

The team carry out annual health assessments for children and young people who are in the care of Bradford Local Authority. They will work with children and young people to develop health actions plans. The aim of this is to ensure all health needs are or can be met. The team can provide support about a variety of health topics or issues and refer or signpost to the right service, if more support is required.

When are the team available?

The team are available Monday to Friday 8.30- 5pm (excluding bank holidays).

Support for young people aged 16+

If you are 16 years or over, you may have been offered a digital version of your health passport using the digital platform Patient Knows Best (PKB). If you have this you can message the team directly.
