If you are a child or young person in care in Bradford and aged 16 and over, you can register for an online health passport- a record of your health history.
Your health history is important. Health professionals may need to view a record of your previous care and treatment so they can provide safe and effective care and treatment for you now and in future.
In the past, young people aged 16 and over were given a paper ‘heath passport’ with a record of their health history. We know these often got lost or damaged. That’s why now, if you are 16 and over, you might have been invited by your BDCFT Child in Care Nurse to register for an online health passport.
Having an online health passport means you can easily
As it is is a digital record, it reduces the risk of any information being lost or damaged. It also enables the service to be more environmentally friendly!
Talk to your named Child in Care Nurse. They will ask you for your current email address and set the account up. You will receive an email invite to activate your account. Having an online health passport is free, quick and easy to do.
Once you have registered you can log in to your online health passport anywhere and at any time, using a smartphone, tablet or computer with an internet connection.
Your Child in Care Nurse will also be able to look at your health passport online. This helps you both to work together to improve your care, using your up-to-date health information.
Keeping your health information safe and secure is very important to us.
If you are a patient of Bradford District Care NHS Foundation Trust (BDCFT), we are responsible for your data. We only share information with the people who need it to provide your treatment and care. If you choose to register for an online health passport, only authorised people involved in your treatment and care will be able to see your data. You can choose to opt-out at any time.
We use Patients Know Best (PKB) software. This meets NHS requirements for health record systems and is compliant with the General Data Protection Regulation (GDPR).
Only the minimum level of data needed will be uploaded. All personal and health information stored is encrypted.
Yes. you can only use one unique email address per registration. If you share an account with other family members, or your carer, they may already use that email for their own Patients Know Best (PKB) account. If so, you will need to create your own personal email account. If you register for an online Health Passport using the NHS app, it will automatically use the email address linked to that account.
Yes. This is an online service and will require you or a carer to have internet access to use your online health passport.
Some people may not have any information in their records when they first register, however this will change over time as you are in contact with the Children in Care Team. You can still add your own information, such as tracking your symptoms or recording any medication you take.
It is your choice if you want to access your health information online. You can simply ignore invitations to register if you choose not to use this method.
If you want a family member or carer to have access to your online health passport, they can be added in the ‘sharing’ section. This will allow them to set up their own account. You can control the level of access they have to your data.
