Language around special needs can mean different things to different people. In Bradford, our team follows on from the Health Visiting Team to support children and young people aged 4-19y who are attending specialist schools across Bradford, focusing on individual strengths, wellbeing, and inclusive care. This includes people with a wide range of additional needs such as physical, learning, social, emotional, and mental health needs.
We are a specialist team made up of admin support, health care support workers, Learning Disability Nurses, Children’s Nurses and General Nurses. The team is run by Bradford District Care NHS Foundation Trust (BDCFT) and can offer advice and support all year round, so even in the school holidays.

You might still remain under the care of our team, depending on where you move schools. If you finish school and choose to attend Bradford or Shipley College though, for example, you will transfer over to the care of the School Nursing Team Bradford School Nursing | Health for Teens | Bradford
