Who we are?

We are a team of specialist nurses, registered nurses and community nursery nurses that support all children in care up to the age of 18 years. We carry out annual health assessments and work with children and young people to develop actions plans to ensure all health needs are met. We talk to children about a variety of health topics and make sure that they are seeing the right person to support their needs.

If you are 16 years or over you may have been offered a digital version of your health passport using the digital platform Patient Knows Best (PKB), if you have this you can message the nurses directly.
